Corrective ActionPlanning整改计划

· Corrective Action Plan (CAP) is a document prepared toensure that the non-compliances reported are corrected.

整改计划是为确保不合规项被整改的文件。

· An effective CAP will include improvements made in theexisting practice/procedure to prevent suchoccurrences from

 repeating.

有效的整改计划包括在现有的体系/操作上改善以防止违规的重复发生。


e.g., Aisles/Emergency Exit Path blocked withWork in Progress (WIP)

如:走廊 /紧急出口通道工作时间被堵塞

NO:

“The materials were removed and the aislecleared. ”杂物已清,走廊已打扫

YES:

“Root cause of materials blocking the aisleis a regular occurrence resulting from production in the area with no temporarystore. Areas near sewing section will be created to store materials in advance oflabeling and packing. All workers in the

sewing section will be informed of the newstorage area and will be directed to place items in the specified location

uponcompletion……..’’

造成出口堵塞的根本原因是车间没有临时的储藏空间导致杂物堵塞走廊;实施整改后,针车课附近将设立储藏空间,并通知员工把物料堆放在指定位置。


Elements of a CAP整改计划的要素

· Root cause on why the non-compliance occurred

导致不合规的根源

· How the same would be addressed

同类问题怎样整改?

· Who is responsible and accountable

谁负责?

· Monitoring mechanism

监督机制

· Written plan – non-compliance, legal/standardsrequirement, plan,timeline, responsibility, review mechanism

书写计划 -不合规项目,法律/标准的要求,计划,时间表,责任人,检查机制


Root Cause Analysis(RCA)根源分析

· Ask ‘WHY’ several times: Normally 5 whys lead to RCA,but not defined. The attempt is to ask why until aconvincing

answer is received.

多问几次“为什么”?通常5个为什么可以察觉但不能确定事故原因;可以尝试多问几个为什么直到获得令人信服的答案。

· The reason might not be only with the observation, thecause may be elsewhere, e.g., OT hours related to latearrival of

raw material,delayed approvals, change in style, etc.

根源并非只是表面看到的,也许还有别的原由,例如加班有时是因为原材料来迟,批准推迟,款式更改等导致。

· Fish bone is a tool commonly used because this gives abroader picture.

鱼骨图通常用来做根源分析,它能比较全面的展示事故原因。
